The Haven in Woodbury, Shakopee, and Waconia provide case management services for clients who have substance abuse issues and may be experiencing other complicating factors in their life that are not fully addressed with traditional treatment approaches.
Case management at Crescendo Chemical Health provides flexible services that meet the emotional and logistical needs of the individual by reducing barriers that may be problematic for recovery or stabilization.
Case management may include:
- Individual substance abuse treatment sessions
- Group substance abuse treatment sessions
- Coordination of social, medical, employment, educational, and mental health services and care
- Assistance with locating and obtaining sober housing
- Transportation to appointments if needed
Services are tailored to the individual’s needs to improve and/or enhance the individual’s support system and to increase the quality of life.
